WASHINGTON – A new report by the U.S. Government Accountability Office confirms some costly concerns regarding the collection of third-party payments owed to the IHS by insurers. Some tribal affairs experts say IHS’ laxity has ended up shortchanging tribal governments and the health of tribal citizens.
One of the report’s most serious findings is that the agency’s procedures for collecting monies from private insurers are muddled.
IHS largely follows federal standards, the report found, but the agency’s overall collection monitoring is inadequate, some financial systems have not been updated, and agency offices are operating under different debt collection plans.
The report found that the problems have likely correlated with IHS not maximizing its collection of money owed by third-party insurers, which it is supposed to do under law.
The Indian Health Care Improvement Act authorizes the agency to collect reimbursement for services provided at IHS facilities from third-party insurers, including Medicare, Medicaid and private companies.
How much money has been left off the table?
In May 2008, Sen. Jon Tester, D-Mont., wrote a letter to then-IHS Director Robert McSwain regarding the problem, which has long been raised by tribal advocates because they have feared that health care for tribal citizens has been compromised.
McSwain estimated IHS collected about 60 percent of what it was owed in fiscal year 2007, which he equated to about $90 million that year.
From that number, it can be reasoned that IHS failed to collect a total of $60 million in 2007 alone.
While the GAO report doesn’t get into deducing those kinds of specific numbers, some tribal affairs experts have already done so, including Tom Rodgers, a Blackfeet citizen and lobbyist with Carlyle Consulting.
“There is an incredible need to be able to assess and quantify the number of dollars at stake,” Rodgers said.
“Tribal governments need to understand the scope of the situation, so they can fight to get these critical dollars returned to them and really improve the health of their people.”
Rodgers has long paid attention to what he believes have been lax IHS collection procedures because he’s been worried that tribal citizens have been shafted due to lack of IHS funding – a well-known and perennial problem. He has also helped the Senate and GAO investigators in their efforts to understand the extent of the situation.
Sen. Max Baucus, D-Mont., chairman of the U.S. Senate Finance Committee, is one legislator who has been especially concerned.
“Right now, American Indians and Alaska Natives are suffering because IHS lacks adequate resources to provide the quality care they need. …” he said upon release of the GAO report, which he requested in early 2008.
“These lapses are unacceptable. I will be tracking the improvement effort carefully and will demand a follow-up report by GAO.”
Baucus also said that current IHS Director Yvette Roubideaux has told him that changes are being made to improve IHS’ collection of payments.
Along those lines, the GAO made several recommendations, including calling for IHS to develop tools for managing and monitoring its payment collection and debt management.
The IHS press office would not offer specific comment on the report, but did note that agency officials have reviewed and agreed with it.
The report noted that agency officials contributed to it and said IHS has already taken steps to ensure the accuracy of its reports and debt management procedures by increasing its oversight capabilities, implementing a new Web-based monitoring service and giving direct line authority to a new position entitled the Deputy Director of Field Operations.

I know Julie Bemer is pre-selecting for positions, a friend who is highly qualified for a position there applied and was told by area office they were not selected but was the best qualified, and that the final decision was up to IHS in Poplar, the CEO who is Julie. , but over a week before this person received the notice from Area they already were told who was selected fro the position. If thta is not preselection then what???? Julie needs to eb removed.

Sandra Schildt is and has always been a true professional in her assessments of IHS and tribal clinics. She does not mention names of clinics and/or staff so if you feel she is talking about you or your clinic, then maybe it's your own guilty conscience talking and your own actions that is "ripping off" your own people.

Who is complaining is all i am saying is why try to get more money in when the money in already isnt being spent right. i agree with third party billing but who is it being spent on is all i think that a closer accounting should be looked at because at our ihs it isnt being spent right and the people are paying for it. It like you can show a good show in the window but look behind the scene. and who is running the show? that what should be asked. there is no complaining only facts!!!!

the SUD in poplar is preselecting for positions and the person is not qualified i thought that pre-selection of postions is illegal. the IHS director does nothing about this. She is putting her friends in high positions with no regards for the people. Now she is trying to run off an indian in a job to replace her with her non indian friend. IHS doesn't bill as much as they can because they have too many people who have no knowledge regarding third party income. It also doesn't help that a lot of people may be eligible for insurance programs and refuse to apply.

If the ihs quits misspending there money, and works harder on getting reimbursed they can take care of business. Alot of people are not being taken care of because the Area Director in Billings,Montana doesnt care, he lets the SUD get away with alot of personal issues and misspending just like everyone else says. This is sad that we depend on them and they are not doing there jobs, what did they do with the stimulus money. Everyone should be able to find out.

i work for the poplar ihs and agree, we loose doctors because of our sud, she has her favorites and doesnt care about her staff and if you talk to her i falls on deaf ears. the tribal board has asked her several times to step down and she refuses. someone comes from billings and tries to cover up for her mismanagement. ms. roubideaux please help us. talk to previous staff that left and you will see. Our health care depends on you. Our sud needs to be transfered out of here.

Our IHS in Poplar Montana is has mismanaged stimulus money and mismanaged funds by the Sud director who sends her non indian staff on workshops with our money to florida, california to have fun. when we are so short of funds for our people. A contract care person that is a fugative from the law for beating an elderly women up is manageing and saying who's bills get paid. she is a friend to the SUD director and gets away with anything. Our IHS dollars are wasted on 2 lazy health educators.

Sandra Schildt doesn't do anything unless it can pad her own pocket. Her reputation proceeds her so that's probably no one is listening to her. Contact the Service Unit she worked at and they will tell you what a con artist she is. Just because she's not from that area doesn't give her the right to rip the Paiutes, Shoshone and Washoe people off.

Sandra Schildt has monitored and developed training and has written to everyone including President Obama and has yet to have anyone recognize her efforts. Maybe someone should take a look at her passionate efforts to improve health care and increase revenue even if she isn't a PHD or someone' buddy in IHS.s

Anybody dealing with IHS contract health or insurance issues already knows the competancy levels of reservation IHS offices and collection efforts. Higher up the ladder you go, mulitply the incompetancy by 10.
